This hospital resource webpage provides access to paternity information and the Hospital Opportunity for Paternity Establishment (HOPE) program, general information and pertinent forms and brochures. The webpage also has a link for ordering forms.

The HOPE program is conducted by Illinois birthing hospital staff and administered by Healthcare and Family Services’ Division of Child Support Services.  For more information on the HOPE program, see the HOPE Fact Sheet.

General Information

  1. Only hospital employees should e-mail the Department of Healthcare and Family Services' Division of Child Support Services. Please include in your inquiry your name, hospital and phone number should we need to contact you.

  2. Frequently Asked Questions

  3. Medicaid/Provider Number – The supply order form requires a provider number. This number is the hospital’s Illinois Medicaid number, which may be obtained by contacting the hospital’s billing department. When entering the number do not use punctuation such as hyphens, parentheses or periods.

  4. Out-of-Stock Forms

  5. Public Health/Vital Records Information/Birth Certificates – Birth certificate/record questions should be referred to the Illinois Department of Public Health (IDPH), Division of Vital Records at 217-782-6554 or the IDPH Vital Records Website.

Healthcare and Family Services’ Division of Child Support Services supplies forms and brochures to hospital staff who assist unmarried parents in establishing paternity for their child.  Hospital staff may access fillable forms as well as order forms and brochures online by using the links below.
