You have been selected to screen with the Illinois Department of Juvenile Justice. This section is ONLY for those who have received a written letter, or an email regarding a screen date. There are numerous forms that you will need to fill out and bring to screening. Please read all information before completing forms to ensure that forms are filled out properly.

You must bring copies of the materials listed below. There will be no exceptions.

  • Driver’s license-you must bring a copy as well as the original
  • Birth Certificate-copy
  • College Transcripts-unofficial copy is acceptable as long as it states degree awarded
  • Selective Service number if applicable (Males 26 and under)

This is a detailed review of how the forms will need to be filled out and brought to screening. Please read all information as it is important everything is completely and properly filled out in order to screen.
 

The first form is the CMS 100. This will need to be filled out and signed. When you are filling out the CMS 100, you will need to list all jobs/schooling/unemployment from the time of your 18th birthday or high school graduation. There should be no gaps of time. For example, if you were employed fill out appropriate sections on the CMS100; if you were in school write school and gap of time on gap sheet; and, if you were unemployed put unemployed and the gap of time on the gap sheet.  We have included instructions for the CMS100 below for you to view, a gap sheet for gaps of time between employment, and a CMS100 form. It is important that there are no gaps of time from your 18th Birthday or high school graduation.

The request for background (DJJ 0032), and the applicant information sheet (DOC 0031), must be filled out accurately, and honestly. The Request for Background Investigation (DJJ 0032) fill out Subject, Other Names Known by, Address, City, State, Zip, States or Countries in which subject has resided, SSN, DOB, and Sex. The Applicant Information Sheet must be filled out completely except for on the last page where it says witness. That will be signed at the time of screening. Make sure to list anything you may be in question about. Information not provided can come up as giving false information when running a background check.

Lastly, we will have you fill out Employment Reference Check forms (DJJ 0037), Employment Release and Consent (DJJ 0035), and the authorization for background check (CANTS CFS 689). For the employment reference check (DOC 0037) you must list the name, address, and phone number of your past/present employers. You will have to fill out an Employment Reference Check (DJJ 0037) form on ALL jobs held in the past 5 years. Special note if applicable, if you have had less than 3 different jobs in that 5 year span you must go back beyond the 5 year time window to show 3 jobs held. You will need to fill out an Employment Release and Consent form (DJJ 0035). Print name, DOB, SSN which is located at top. Sign/Date at the bottom where it says applicant signature. You do not need a witness signature this will be signed at screening. The authorization for background (CFS 689) must be completely filled out. Do not mail off any of these forms. We will collect them at the time of screening.

Reminder: Once again, please remember to bring all forms listed above fully and completely filled out. All forms must be accurately filled out. There are 7 forms all together. ERC’s will vary depending on the applicant. Also bring all required copies listed. Without this information you will be unable to screen.
